About the Institute:
Sher-i-Kashmir Institute of Medical Sciences was conceived with the following objectives:
To provide facilities of specialized medical care and particularly develop super specialties that would provide tertiary health-care. To provide need oriented education in medical sciences and clinical research. To develop a referral linkage between the primary, secondary and tertiary health-care Institutions of the State to achieve an optimum health delivery system. The State Government under the 5th and 6th plan grants earmarked for the Institute, as the planning commission approved of it as a plan project, provided the funds for construction and equipping the Institute.

The construction commenced in the year 1976. The Institute was partially commissioned on 5th December 1982.Sher-i-Kashmir Institute of Medical Sciences is a post graduate Institute for training, research and patient care. With this objective, various committees appointed by the Government of Jammu and Kashmir identified the specialties in which postgraduate and post doctoral courses would be undertaken. By an act of Legislature on 19th August 1983, Institute of Medical Sciences was granted a deemed University status.

IN Patient
The Inpatient services at the Institute comprises of general admitting Wards for the Clinical Department, Intensive Care Units and other Intermediate care areas. There are Seven Floors each comprising of two independent wards and two floors for Paying Wards. In addition there are areas for Post Transplant, Hematology-Oncology and Burns patients.

OUT PATIENT
The Outpatient facility comprises of a poly clinic were in patients are screened and filtered for further assesment in the Refferal Clinic. The Refferal Clinic is run by various Super-speciality departments. The Refferal Clinic also has an independent area for Sample Collection, Basic Investigative Setup and Dietary & Psychosocial counselling.

Emergency
Emergency service at SKIMS has a three-tier system: Filter Clinic, Emergency Reception and Emergency Recovery. This 24 hour Service caters to all patients who are either victims of road accidents, violance, medical emergencies as well as critically sick patients reffered from other sister institutions.

Community Services
Department of Community medicine runs rural health services in the Hajan Block of North Kashmir. Fully integrated health center has been established which provides primary health care to the local residents there. In addition immunization and health education programs are run there.

The CSSD is a vital service supported department of the hospital. This department renders service to all patient care areas. The main aim of the department is to give sterilized supplies to all the areas of the hospital. The department reprocesses all the soiled surgical and diagnostic items which are being used in different areas for washing, sorting, assembling, packing, sterilization and for issuance to the user areas of the hospital.

All the departmental working activities are done by the well trained technical staff and these activities are monitored and supervised by the CSSD Superintendent under hospital administration. The department provides the following services to the hospital round the clock.

  • Disposable syringe services
  • Dressing material services
  • Surgical gloves services
  • Diagnostic and surgical packs
  • Ethylene Oxide Gas re-sterilized packs
  • Collection and delivery system by giving trolley service
  • Short training courses to Nursing College Students, B. Sc. and M. Sc. Lab and Theatre Technology Students.

Dietetics is a field, specializing in Therapeutic and Clinical nutrition. The dietetic profession is expanding work in multi-disciplinaries to provide wholesome health care to patient. Dietetics and Therapeutics department is one of the most important hospital services contributing to the recovery of health through scientifically prepared diet. This department contributes towards the basic needs for good nutrition for the patient.

Activities:
In-patient food services, Nutrition & Diet Therapy Consultation and counseling, and teaching Post-graduate students of Nutrition, dietetics and food service management are the main activities, this department is involved with.

Food Service Management:
The whole Dietetics and Therapeutic department is consciously involved to ensure that the prescribed diets are given to the respective in-patients. Our team of dietitians plan the meals and cycle menu according to the seasonal availability of food stuffs and also as per the approved dietary scales. Our team of professionally trained cooks does the mass production of cooking of healthy meals hygienically and the service staff renders hygienic and in-time service to inpatients under the guidance of qualified supervisory staff and Dietitians.

Recruitment and promotions :
The vacant post of Chief Manager of D & T was filled in this year Dr. Vasanta Kohli assumed the charge of Chief Manager & HOD in this year.

Academics
The department is recognized to impart internship training to students pursuing P.G degrees in Dietetic & Clinical Nutrition. During this year training of four batches of IGNOU students and one batch of Kashmir University students were undertaken. Many Nursing students & students from Kashmir University receive practical training in the Dietetics and Therapeutic department. Also theory classes of B.Sc Nursing are being taken by the officials of the department.

The department provided the expert guidelines and helped the Directorate of social welfare, Kashmir for revising the Nutritional and feeding norms for supplementary Nutrition in ICDS scheme for the year 2009.

Dr. Vasanta Kohli and Mrs. nasreen Paul were appointed as examiners by Kashmir University for conducting practical & Theory examination for PG student of Nutrition and Dietetics.

During this year two research proposal have been undertaken by the department .

  • Dietary trends in Kashmir with reference to Srinagar city.
  • A study on patients acceptance of Hospital diets and food services with reference to patients satisfaction.
Dr. Vasanta Kohli HOD has attended, the 42th Annual National Conference Indian Dietetic Association 12-13 December 2009. The theme of the Conference was Dieteties in India: The challenges Ahead held at CSM University UP (erstwhile King George Medical College) Lucknow: Chaired scientific session, presented Paper titled. A study of Dietary Treads in Kashmir in the special reference to Srinagar city.

Mrs. Shahana Quershi, Sr. Managar attended a Training Course on Public Health Nutrition 14th-18th December 2009, Conducted by the Department of Community Health Administration at National Institute of Health and Family Welfare, .New Mehurauli Road, Munirka, New Delhi sponsored by the Institute (SKIMS)

Dr. Vasanta Kohli, has been selected by the Kashmir University for Guiding the Research Dissertation of P.G Students (MSc Dieteties and Clinical Nutrituin). A number of students of IGNOU from Jammu and Srinagar are also doing the research Dissertation under the Guidance of Dr. Vasanti Kohli. Mrs. Heena Jr. Dietation is also being appointed for the research Guidance of PG. students of Kashmir University.

Nutrition & diet Therapy consultation & Counseling:
Besides serving nutritious diet to patients, this department also provides this facilities to both in door & out door patients. Calls from wards are attended by Dietitions as and when it is received.

Dr. Vasanta Kohli provides Nutrition consultation to the out patients in Referral Clinic twice in a week on Monday and Thursday (R.NO. 170)

The objective of this service department is to collect all hospital soiled/dirty linen on daily basis from all patient care areas which include inpatient ward block, operation theatres, laboratories and outpatient department and distribution of washed and clean linen on exchange basis for all these areas once daily, washing of hospital employees uniform including aprons of all doctors of institute.

Hospital linen procured and stitched which include doctors aprons, employees uniforms, patient suits and operation theatre staff suits. During times of need for SKIMS Medical College Hospital, Bemina services are provided to that hospital also.

MEDICAL RECORD
The Medical Record department of the Sher-i-Kashmir Institute of Medical Sciences was started in the year June 1982 with the commissioning of the out-patient department. This department has been responsible for the development, preservation & processing of the patient records for so many purposes. Besides this patient billing has been also given as one of the main responsibility to this department. The department is organized as below.

Brief description of this organizational pattern is given as under:

ADMISSION AND ENQUIRY OFFICE
All admissions to the hospital take place through this sub office of Medical Record department. Two types of Inpatient admissions are routed through this (a) short stay admission (b) routine admissions. This office remains open for 24 hours for the said purpose. This office initiates documentation of patients strictly as per admission forms issued by heads of departments through their nominated representatives.

Besides records of all the admissions, discharge, death references as well as it collects documents after discharge and forward the same to the main Medical Record department for further processing. This way it also keeps up to date information of bed state of each ward. Patient billing of admissions, discharges is a regular feature of this area. All the hospital charges of the patients admitted are levied for patient services at this office.

Patient information of hospitalized patients and other hospital activities is being provided form here to the general public and to the general public and to the press under certain procedures. Around 120 patients are being registered here at this office and the same number of patients discharged routinely.

MAIN MEDICAL RECORD DEPARTMENT (CENTRAL RECORD OFFICE)
Following functions are performed here at this area.
  • Receipts, Checking, assembly, storage of all inpatient records of discharged patients routed through Central Admission office.
  • Daily discharge analysis and statistics
  • Coding of all diagnosis as per international classification of disease and health related problems.
  • Indexing of all discharge patients by disease, department, doctor and so on (Computerized)
  • Making Patioent records available for medico-legal purposes to police personnel, courts and attorneys etc.
  • Preparation of Medical certificates and their issuance to the patients or to their next of kin after proper verification.
  • Notification of deaths to the Srinagar Municipal Corporation.
  • Research purposes. Issue of patient records to medical officers, post graduates whenever required for court attendance, abstract presentation, case studies and presentation in the mortality and grand round meetings.
  • Preparation of monthly abstracts and annual statistical details.
  • Dealing with enquiries for life insurance, corporation, other detective agencies regarding disease and cause of death of the insure.
  • Training of various categories of personnel.
  • Storage of all types of forms used in the hospital
OUTPATIENT SERVICES
  • Registration of all the new and old patients.
  • Assuring the maintenance of OPD records in different OPD clinics.
  • Prepares different OPD statistics i.e. disease, sex and visit, new and old basis.
  • Ensures the issuance of medical and other certificates.
  • Maintains the record of investigations of out patients in the Radiotherapy OPD.
  • Patient appointments on first come first serve basis.
  • Patient billing. (Registration and investigations.)
ENQUIRY SERVICES
  • Handling patient enquiries.
  • Providing patient assistance
Responsibility of printing press has been given to this department since long. All the patient printing requirements in terms of investigation requisition and registration forms, registers various types of cards especially case files are being printed and prepared for the patient cause. In the last year i.e. sept.2006 a mini offset press was installed in the printing press in view of its need and requirement.

Last but not least Medical Record training programme for in service candidates has been approved and classes for the first batch are likely to take place in the month of Nov.2007.
